Step 1: Initial Contact Who Can Initiate:

Step 2: Eligibility Screening Determine Eligibility:
Our intake specialists will discuss the veteran’s medical history and current health status to determine eligibility based on the prognosis and other VA-specific criteria.
Documentation Needed: The veteran’s VA and medical records will be requested to verify eligibility and understand medical needs.
Step 3: Assessment Appointment Scheduling the Visit:
An in-person or telehealth appointment will be scheduled to assess the veteran’s medical needs, personal preferences, and family input. Comprehensive Evaluation: This includes a detailed discussion about the veteran’s health, end-of-life wishes, and any specific needs or concerns.
Step 4: Care Planning Developing the Care Plan:
Together with the veteran and their family, our team will create a personalized care plan that addresses all aspects of the veteran’s physical, emotional, and spiritual needs.
Team Introduction: The veteran and family will be introduced to their hospice care team, which may include doctors, nurses, social workers, spiritual counselors, and volunteers.
Step 5: Service Agreement and Consent Review of Services:
We will provide detailed information about the services to be provided, what to expect, and any other pertinent information.
Signing Consent: The veteran or their legal representative will need to sign a consent form agreeing to receive hospice care services.
Step 6: Start of Care Commencement of Services:
Care services will begin as outlined in the care plan. This could be at the veteran’s home, in a nursing facility, or at a hospice center, depending on the agreed-upon setting.
Ongoing Communication: Regular updates will be provided to the family, and adjustments to the care plan will be made as necessary to ensure the veteran’s comfort and dignity.
Step 7: Continuous Support and Adjustment Monitoring and Adjustments:
The care team will continuously monitor the veteran’s condition and adjust care as needed.
Family Support: Ongoing support will be available for family members, including counseling, bereavement support, and resources to help them through the process.
Step 8: Service Initiation Begin the delivery of hospice care services as outlined in the care plan.
